Adminstration Fee:
$25 per session/per family or $75 per program year/per family

JUNE SPECIAL: Admin Fee only $60 per family if you register in June.

Enrollment Form
Each family must complete an Enrollment Form for each session.

Participation Agreement
Each family must complete one Participation Agreement for the program year.

Medical Form
Medical Form is optional, however it is stronger advise if your children are left under another adult's care.
